The Care Team
Professional, caring individuals, from many disciplines, provide health care in Thompson House and work together as a team with the resident and family to ensure an integrated approach to resident care. The expertise of each person on the care team contributes to the comprehensive assessment of resident's needs and collectively they plan, implement, monitor and evaluate the resident's care.
Restorative and palliative care are provided in a positive, supportive manner with emphasis on health and comfort. Members of the care team review the resident's plan of care quarterly and on an on-going basis if the resident's health care needs change. The care team meets with the resident and/or the Substitute Decision Maker to review and plan for the resident's care six weeks after admission, if the resident's condition changes and on an annual basis.
