Community Services: How to Apply
Intake Department
All the programs and services offered through E.P. Taylor Place Community Services may be accessed with a single phone call. Our experienced, professional staff are available Monday to Friday from 8:30 a.m. to 4:30 p.m. to provide you with all the information you need to choose the services that are right for you or the person you are caring for. Once you call, we will begin the process of implementing your service and will connect you with other members of the community services team to ensure your needs are met in an appropriate, timely manner. If you require support that may best be provided by another organization, we will be happy to refer you to another service provider who may better meet your needs.

Community Care Access Centre
Community Care Access Centres (CCAC) are publicly funded community based organizations that may help you with defining your needs and situation, determining your eligibility for government-funded services, locating providers and applying for care. If you are eligible for government-funded personal care and support services, they are arranged by the CCAC and paid for entirely by the Ministry of Health and Long-Term Care. Otherwise, these services may be available directly through providers, like the Don Mills Foundation, in which case there will be a consumer fee for these services.

If you would like to inquire about personal care services, you can call your local Community Care Access Centre directly for more information or ask one of our Social Workers for assistance. If you are not eligible for CCAC hours or you would like to purchase additional hours through our In-Home Services department, we would be happy to assist you.
