Our professional, qualified Social Workers are available to provide information and referrals, assessment of needs, coordination of services and follow up, crisis intervention, long term planning, linkage with community services, financial assessments and family support groups. Each individual's need for community and residential services is evaluated with care and concern and home visits and family meetings are arranged at no cost according to individual requests. Caregiver support groups are offered on a regularly scheduled basis to provide guidance, respite and socialization for people caring for someone at home.
